Life and AD&D Insurance (The Hartford) is offered to employees working a minimum of 30 hours a week 1st of the month following 30 days of employment and is paid for by the Company in an amount equal to one and one-half times your salary to a maximum of $300,000. An additional amount equal to one and one-half times your salary to a maximum of $300,000 will also be paid by the Company for Accidental Death & Dismemberment.
Basic Life and AD&D Coverage is provided by Avalon Flooring at No Cost to You. No action is required - eligible employees are automatically enrolled.

1.5 times your annual salary, up to a maximum of $300,000. Benefit amount reduces by 35% at age 70.
