As a member of the Avalon Flooring health plan administered by Luminare, you can access health benefits information through the secure online portal and through the Luminare Mobile App (available through the Apple App Store and Google Play).
Simply visit www.myluminarehealth.com and register.
You’ll have convenient access to benefits information, as well as resources and interactive tools to help you manage your health and wellness, 24/7.
If you have previously registered with Trustmark, you will not need to do anything differently. This is for new members registering with the portal.
Register for Online Access
1. Go to: www.myluminarehealth.com
2. On the right side of the screen, under “Register”, click to create your Participant account.
3. Follow the instructions to enter your Member ID (or SSN), last name, zip code, and date of birth.
4. You will then create a username, password, and security questions of your choice.
5. Log in and see your ID card, find a doctor or hospital, view your recent claims, and more!

When you register, your personalized account will give you access to important health information for you and your dependents.
